The challenge inherent in establishing effective medication reconciliation processes is persistently obvious. Implementation and satisfaction rates have seen little progress this year. However, more facilities are moving toward automating their processes, and those who have done so are significantly more satisfied than the hospitals that continue to take a manual approach.
